Care.Data/HSCIC/SCR – How information from your medical record will be used in the near future

Care.data/HSCIC – Confidential information from your medical records can be used by the NHS to improve the services offered to provide the best possible care for everyone. This information along with your postcode and NHS number but not your name, will start being extracted to a secure system where it can be linked with other health information. This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you. Your data may also be made available to organisations outside of the NHS, such as universities and commercial organisations. We have no choice but to allow the HSCIC to extract the information.  The Government has had a leaflet regarding this data upload delivered to every household in the UK.

You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please contact the surgery and ask for an Opt-Out Form.

Summary Care Record – Your data has been uploaded to the SCR to enable other hospitals and healthcare providers to view your records.  You can find further information by clicking here.

Note that these are two different data uploads so you will need to complete both forms if you wish to opt out of both data uploads.  Please ask us if you wish for further information or clarification.
